Caregivers were seen using cell phones for personal reasons while supervising infants.

There is sufficient evidence to prove that an infant was in a swing for more than 15 minutes.

An infant was left unsupervised when a caregiver walks out of the room for 2 minutes. Additionally, caregivers are unaware of an infant?s on-going activities when an infant gained access to the sleep only area without intervention by caregivers.

746.5101(b) - Annual Fire Inspection - Conducted by Fire Marshal

The operation has not had an annual fire inspection done in the last 12 months.

746.605(16) - Required Admission Information-Completed Food Allergy Plan

2 children with diagnosed food allergies did not have a food allergy emergency plan on file at the operation.

746.401(5) - Posting Requirements - Activity Plans for each group of children in the child care center

Two rooms did not have posted activity plans. This was corrected when they were found and posted.
